Responsibilities

• Provide direct nursing care to residents in a long-term care setting per the plan of care
• Administer prescribed medications and monitor responses, documenting promptly
• Monitor vital signs, assess changes in condition, and report to the supervising RN
• Assist residents with activities of daily living and mobility; support personal care needs
• Maintain accurate nursing notes and medication administration records in the electronic health record
• Collaborate with the interdisciplinary team to implement care plans and participate in rounds and safety checks

Requirements

• Active Licensed Practical Nurse license in New Hampshire
• Current CPR/BLS certification
• Ability to work 12-hour shifts on a rotating schedule
• Experience in long-term care or geriatric nursing preferred
• Successful completion of required background check and drug screening
• Eligibility to work in the listed setting
