Care Manager (Health Homes) Bilingual Spanish Preferred

Job Code: 2026-036 Location: Brewster Offsite Office Department: Community Base Svcs Status: Regular Full Time (40 hrs/wk) TheCare Manager acts as the key contact for managing and coordinating medical,educational, and behavioral health services for young people and theirfamilies. They work on resolving access issues, building strong connectionswith service providers, tracking progress, and identifying any barriers tocare. By working closely with each family and care team, the Care Manager ensuresthat all providers are aligned in their dedication to the youth's well-being,helping to identify and leverage their strengths, thus empowering them tosucceed both at home and in the community . Education/Experience: Minimum of an Associate’s Degree with two years ofexperience is required Bachelor's Degree with two years of experience ispreferred. Candidates who can demonstrate they are workingtoward a Bachelor’s degree with a graduation date within one year can beconsidered. Relevant expertise and experience in serving children andfamilies in child welfare, developmental disabilities, mental health,healthcare, and/or other systems, as well as those receiving preventiveservices. Demonstrated understanding of the needs of high acuitychildren and their families as evidenced by additional years ofexperience, education, and training. Bilingual is a plus. Licensure/Certification: Valid driver’s license and a registered operating car to beused for daily transportation for work-related purposes This is a full-time and benefit-eligible role.
