Community Case Manager - Empower Independent Living

Comunilife is seeking a Case Manager for its Bronx residential program. You will develop supportive relationships with clients, link them to community-based medical and mental health care, provide crisis intervention, and assist access to primary care to promote independent living. The role requires a Bachelor’s in human services, at least two years’ experience, and familiarity with HIV/AIDS and mental health challenges. #J-18808-Ljbffr Comunilife
