LICENSED PRACTICAL NURSE

divh2LPN Job Post/h2pResponsible for the overall operation and optimal quality of care for the residents on the assigned unit. Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the facilitys administration and the Nursing Policies and Procedures./ppAs an LPN you will be responsible for providing residents assistance with ADLs and in the administration of their medications while overseeing staff. Adhering to the instructions of Nurse Management, you will perform your duties in accordance with all company policies and procedures, while providing resident care services with kindness and compassion to the residents./ph3Responsibilities/h3p Holds daily shift report with nursing assistants assigned to the unit prior to the start of the shift. Reviews written daily assignments for each resident on assigned unit. Assures assignments are completed before the end of the shift/pp Makes rounds on assigned residents at least once every two hours/pp Administers medication and treatments according to the physicians orders. Assumes responsibility for ordering medications/pp Collaborates with Care Plan Coordinator to develop and maintain nursing care plans and update these plans on a monthly basis or more often if the residents condition changes/pp Assists the attending physician with rounds and special procedures as needed/pp Transcribes, documents and implements physicians orders/pp Contacts the residents physician and family to notify them of a change in the residents condition or accident/incident; also notifies the supervisor covering/pp Assumes independent responsibility for yearly written evaluations of nursing assistants and documents accurately their strengths, weaknesses and the steps taken to improve their performance. Such evaluations become part of the nursing assistants permanent records and shall be used as the basis to determine the continued employment of nursing assistants, promotional opportunities and the amount of any wage increases granted/pp Assumes independent responsibility for documenting disciplinary concerns for nursing assistants on designated shifts, including reprimands, oral and written warnings, and recommending the suspension or discharge of an employee/pp Assumes independent responsibility for completing required routine documentation and auditing as required/pp Participates in the hiring, interviewing and orientation of nursing assistants/pp Assist in the Quality Assurance and Performance Improvement (QAPI) process/ph3Qualifications/h3pstrongEducation / Experience:/strong/pp Practical Nurse licensed in the State of employment/pp Minimum one year of education or experience in geriatric nursing or rehabilitation preferred/pp Knowledge of nursing theory and practice, including the administration of medications/pp Ability to read, write and comprehend English/pp CPR certified/ph3Supervisory Responsibilities/h3p This position has no supervisory responsibilities./ph3Work Environment/h3p This job operates in a health care setting. This role requires regular walking to various locations around the care center./pp Incumbent may be exposed to virus, disease and infection from patients and specimens in working environment./pp Incumbent may be required to work extended periods of time at a video display terminal./pp Incumbent may experience traumatic situations, including psychiatric, and deceased patients./ph3Physical Requirements/h3p The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job./pp While performing the duties of this job, the employee is regularly required to talk and hear.
