Your Role:
- Deliver personalized physical therapy services to home-based clients.
- Conduct initial and ongoing assessments to track progress and adjust treatment plans.
- Perform fall risk assessments at Assisted Living Facilities (ALFs) and community expos.
- Supervise client care to ensure best practices and quality outcomes.
- Represent our organization at ALF events and expos, providing educational presentations.
- Travel to client locations and events as needed.
- Supervise Home Care clients and caregivers
- Create and update care plans for non-medical clients
- Administrative duties including calling PCP offices for orders
What You Need:
- Licensed Physical Therapist (PT) in the state of Hawaii.
- 1+ years of experience in a clinical or home-based care setting.
- Experience with fall risk assessments (preferred).
- Excellent communication and interpersonal skills to engage clients and the community.
Valid driver's license & reliable transportation to travel as needed.
