Summary:
The ACS/Nephrology Clinical Pharmacy Specialist is a member of the Acute Care Services team and is assigned primary responsibility for providing comprehensive pharmaceutical care to patients admitted to the acute care service line and the Nephrology service. The ACS/Nephrology Clinical Pharmacy Specialist collaborates with other pharmacists, physicians, nurses, and other healthcare professionals to ensure the delivery of safe, efficient, and comprehensive medication therapy management.
Responsibilities:
- Daily inpatient rounding on Nephrology teaching services and clinical support on assigned unit(s)
- Operational and clinical pharmacy support for Nephrology outpatient and clinic areas
- Participates in the planning, development, and monitoring of patient treatments
- Investigates therapeutic alternatives and recommends or initiates the management of patient-related problems based on interpretation of relevant literature and clinical experience
- Works with other health care providers and relevant committees to develop programs for improving drug use and quality of care including development and management of pharmacotherapeutic protocols
- Directs development, implementation, and maintenance of clinical pharmacy services in the specialty area
- Routinely reviews biomedical and pharmacoeconomic literature relevant to the pharmacotherapeutic management of patient populations
- Provides leadership/mentorship to the clinical pharmacists and pharmacy residents
- Assumes responsibility for the education of all members of the health care team involved in patient pharmacotherapy
- Serves as preceptor to pharmacy students, residents, and fellows
- Routinely provides drug information and presentations to the patients/caregivers, medical, nursing, and pharmacy staff
- Identifies therapeutic categories and individual agents warranting drug utilization review
- Develops and conducts drug utilization evaluation in these targeted areas and presents data to appropriate hospital committees
- Develops, implements, evaluates and participates in scientifically valid and ethically designed studies
- Presents research results at professional meetings and/or publishes results in the professional literature
- Works collaboratively in development, implementation, and ongoing review of Epic order sets
Work Experience:
- Minimum of three (3) years of prior hospital experience required; completion of a one year residency is considered three years of experience
- Must meet PGY2 preceptor eligibility requirement as define by ASHP
- Completion of a manuscript suitable for publication preferred
Education:
- Doctorate of Pharmacy (PharmD) degree required
- Completion of a pediatric clinical pharmacy residency or equivalent clinical experience required
- Completion of a 20 hour ACPE accredited IV training course required within 90 days of hire
Certifications:
- Current Texas Pharmacist license
- Basic life support for Healthcare providers as required by CP 1.20 Life Support Course requirements
- PALS Certification required for all emergency care and critical care pharmacists
- ACLS Certification required for all emergency care and critical care pharmacists
- Board of Pharmacy Specialties Certification in specialty area required within 2 years
- Texas State Board of Pharmacy preceptor certification within 90 days
