We are a multi-specialty healthcare organization dedicated to improving care for residents in skilled nursing facilities. Our clinicians provide essential medical and behavioral health services that support facility care teams and ensure patients receive consistent, high-quality care.

Founded in 2018 and headquartered in Chicago, we partner with skilled nursing facilities across the Midwest and East Coast. Our providers deliver integrated clinical services including internal medicine, psychiatry, psychotherapy, and podiatry, helping facilities meet the complex medical and behavioral health needs of their residents.

Position Summary

Vitae Health is seeking a qualified and compassionate Podiatrist to provide comprehensive podiatric care services to residents in a skilled nursing facility. The Podiatrist will be responsible for evaluating, diagnosing, and treating conditions of the foot, ankle, and lower extremities. This role plays a critical part in promoting mobility, preventing complications, and enhancing overall quality of life for residents through proactive and specialized foot care.

Key Responsibilities

  • Provide comprehensive podiatric evaluations, including assessment of skin integrity, vascular status, neurological function, and musculoskeletal conditions
  • Diagnose and treat a wide range of foot and lower extremity conditions, including infections, ulcers, deformities, and nail disorders
  • Perform routine and advanced podiatric procedures such as nail debridement, callus reduction, wound debridement, and minor surgical interventions within scope of practice
  • Develop and implement individualized treatment plans, with a focus on prevention and management of high-risk conditions such as diabetic foot ulcers and pressure injuries
  • Provide and oversee wound care treatment for lower extremity wounds in collaboration with nursing and wound care teams
  • Monitor and manage chronic conditions affecting foot health, including diabetes-related complications and peripheral vascular disease
  • Collaborate with interdisciplinary team members including nursing staff, rehabilitation therapists, and primary care providers to ensure coordinated and holistic resident care
  • Participate in care plan meetings and contribute to interdisciplinary treatment planning
  • Educate residents, families, and staff on foot care, preventive measures, proper footwear, and management of chronic conditions
  • Maintain accurate, timely, and compliant clinical documentation in accordance with facility, state, and federal regulations
  • Ensure adherence to all infection control standards, as well as HIPAA and OSHA requirements
  • Provide clinical oversight and guidance to podiatry assistants or support staff as applicable
  • Assist with scheduling, documentation workflows, and coordination of podiatry services within the facility
  • Documentation must be completed within 72 hours of encounter completion.
  • Provide services to 80% of facility census.

Requirements

  • Doctor of Podiatric Medicine (DPM) degree from an accredited podiatry program
  • Active and unrestricted state license to practice podiatric medicine
  • Board certification or board eligibility preferred
  • 4-5years of experience in podiatry, with experience in long-term care, geriatrics, or wound care strongly preferred
  • Strong clinical knowledge of lower extremity pathology, diabetic foot care, and wound management
  • Experience performing minor surgical procedures and wound debridement
  • Familiarity with infection control protocols and best practices in a clinical setting
  • Excellent clinical judgment, diagnostic, and problem-solving skills
  • Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and interdisciplinary teams
  • Ability to manage time efficiently and adapt to a fast-paced clinical environment
  • Proficiency with electronic health record (EHR) systems preferred
