Primary Care Physician-OSH

Role: Provides equitable, value-based primary care to Medicare patients in a neighborhood clinic setting, focusing on delivering high-quality, outcome-driven healthcare that emphasizes prevention and reduces hospital admissions. Responsibilities: Assess and diagnose patients, oversee primary care services, prescribe medications, maintain electronic medical records, collaborate with leadership to meet health quality goals, manage care teams, and perform other duties as assigned. Qualifications: MD or DO, board-certified or eligible in Internal Medicine or Family Medicine, active unrestricted license and DEA, US work authorization. Preferred: geriatrics fellowship, experience with older adults, value-based care, outpatient primary care, bilingual skills. Skills & Benefits: Strong clinical, collaborative, and leadership skills.
