Responsibilities The Home Health Occupational Therapist (OT) assesses and evaluates patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, adaptive equipment, and other occupational therapy requirements based on medical conditions. Provide services within the scope of practice defined by state laws, in accordance with the plan of care, using evidence-based techniques, and in coordination with the healthcare team. Evaluate the patient’s functional status, body system status as required for CMS documentation, and occupational therapy needs. Consult with the physician to develop the therapy plan of care. Observe, record, and report the patient’s response to treatment and any changes in condition to the nurse supervisor and physician. Conduct patient assessments evaluating functional ability through diagnostic and prognostic tests. Education and Experience License Requirements Current Occupational Therapy license in the state of practice Current CPR certification Valid driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation
