NOW HIRING: SUPPORT SERVICES & PERSONAL CARE LIAISON ????

Independence Home Health LLC is looking for a dependable, compassionate, and customer-service-driven Liaison to join our team!

The Support Services & Personal Care Liaison will help build strong relationships with clients, families, caregivers, and our office team while coordinating and supporting services that meet each client’s needs.

???? POSITION QUALIFICATIONS & RESPONSIBILITIES

• High school graduate
• Minimum of six (6) months of experience with a regulated personal care agency or similar company
• Maintains current CPR certification
• Ability to supervise and direct care team members
• Excellent verbal and written communication skills
• Strong interpersonal skills with the ability to delegate and communicate tactfully with clients, families, staff, and the community
• Proficient with Windows-style applications and software programs specific to the role
• Excellent time-management and organizational skills
• Ability to mentor and support agency staff
• Maintains a valid Indiana driver’s license as required by state and/or agency policy
• Maintains access to a reliable, insured automobile in good repair that meets state and/or agency requirements and is capable of fulfilling staff and/or client transportation needs while on duty
• Eligible to participate in Medicaid programs
• Must agree to required background screenings, including driving record, county/federal criminal background checks, national criminal search, and Sex Offender Registry check
• Available 8:00 AM–4:30 PM and some evenings for required meetings
• Available during regular business hours in person, by phone, or through another agency-approved method of communication

???? KEY RESPONSIBILITIES

• Perform non-clinical/medical admissions of clients into the personal care agency
• Coordinate and collaborate with the Program Manager, Scheduling Coordinator, Biller, field staff, informal caregivers, and other individuals or entities involved in the client’s care
• Support both Support Services and Personal Care programs
• Help coordinate services and respond to client and caregiver needs
• Maintain positive relationships with clients, families, caregivers, and community partners
• Supervise and provide direction to care team members as needed
• Communicate effectively with the care team to help ensure client needs are addressed
• Provide ongoing support, follow-up, and exceptional customer service
• Assist with identifying and coordinating additional services or resources when appropriate
