Home Health Registered Nurse (RN)

Location: Florida (specific city not disclosed)

Assignment Duration: This is a 13-week contract position.

Schedule: Day shift, 5 days a week, 8-hour shifts (08:00 - 17:00). Every 3rd weekend required.

Job Description: We are seeking a compassionate and dedicated Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes. This role requires strong assessment skills and the ability to document care effectively while ensuring patient safety and education. Experience with a wide range of patient populations, including cardiopulmonary, geriatrics, neuro, ortho, and oncology, is essential.

Responsibilities:

• Conduct patient assessments and create personalized care plans.
• Administer medications and manage medical equipment in patients' homes.
• Educate patients and their families regarding health conditions and care plans.
• Monitor patients for changes in condition and respond accordingly.
• Ensure compliance with safety and health regulations.
• Complete documentation accurately and timely via the assigned system.
• Provide case management support and coordinate care with interdisciplinary teams.

Required Experience / Certifications / Licensure:

• Minimum of 2 years of experience in Home Health Care.
• Current State Registered Nurse License (Florida or compact).
• Basic Life Support (BLS) certification required.
• OASIS training preferred.
• Must have a valid driver's license and reliable transportation for home visits.
• Must be COVID vaccinated or have an approved exemption.
• Flu shot required or signed declination, with mandatory mask wearing while at work.
