Assisted Living RN

Key Responsibilities
  • Oversee and coordinate comprehensive medical and nursing services for Assisted Living Program (ALP) residents to ensure high-quality, person-centered care.
  • Recruit, screen, train, supervise, and performance-evaluate Certified Home Health Aides (CHHAs) and Personal Care Aides (PCAs) in alignment with NYS regulations.
  • Perform resident assessments, determine care needs, and approve admissions based on clinical suitability, staffing capacity, and program requirements.
  • Review, approve, and sign all required clinical, regulatory, and documentation (including UAS-NY assessments where applicable) to maintain compliance and accuracy.
  • Ensure full adherence to New York State regulations, facility policies, quality assurance standards, and best practices in long-term care.
  • Maintain precise, up-to-date records for residents, staff, and program operations, including electronic health records and reporting.
  • Actively participate in program development, quality improvement projects, complaint investigation and resolution, and interdisciplinary team efforts.
  • Provide direct support to residents, staff, and operations across all service areas as needed to promote resident well-being and program success.
  • Partner closely with the Administrator on operational planning, resource allocation, and effective care delivery strategies.
  • Lead by example, demonstrating professionalism, strong organization, proactive follow-through, and a commitment to team excellence.
  • Perform other duties as assigned by the Administrator or senior leadership.
Qualifications / Requirements
  • Active, unrestricted New York State Registered Nurse (RN) license required.
  • Associate's or Bachelor's degree in Nursing or a related field from an accredited institution.
  • Minimum age: 21 years.
  • Experience with the Uniform Assessment System (UAS-NY) highly preferred and considered a significant advantage.
  • Prior experience in home care, Assisted Living Program (ALP), assisted living, long-term care, or skilled nursing strongly preferred.
  • Proven leadership, supervisory, and team-building abilities with a track record of motivating, training, and evaluating direct care staff.
  • Exceptional verbal, written, and technical communication skills for interacting with diverse stakeholders.
  • Proficient in Microsoft Office (Word, Excel), electronic health/medical records systems, and relevant software tools.
  • Highly organized with excellent time management, prioritization, and multitasking skills in a dynamic environment.
  • Strong critical thinking and sound judgment, with the ability to balance resident rights, regulatory compliance, and operational needs.
  • Professional, compassionate demeanor when engaging with residents, families, staff, regulators, surveyors, and the broader community.
