Description

• Assess patients in both inpatient and outpatient environments and develop personalized treatment strategies.
• Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functionality.
• Work together with nurses, doctors, case managers, and other rehabilitation professionals to achieve optimal patient outcomes.
• Guide patients and families through understanding therapeutic objectives, discharge processes, and at-home exercise routines.
• Maintain accurate documentation of evaluations, treatments, and outcomes following facility and regulatory standards.
• Engage in care team meetings and interdisciplinary discussions to ensure cohesive patient care.

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution.

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Additional specialized certifications (e.g., neurological, geriatric) are advantageous, though not mandatory.
