Required Licenses

Active Arizona Nurse Practitioner license OR
Active NP Compact License (proof of active compact required)

Required Certifications

ANCC or AANP Certified

Highly Preferred

Active BLS Certification
Active DEA
Experience in Palliative Geriatrics

Practice Details

Outpatient clinic setting
Supporting a Palliative Care team focused on geriatric patients
Manage chronic conditions including dementia, arthritis, diabetes, and cardiovascular disease
Conduct comprehensive health assessments and cognitive screenings
Medication management with emphasis on reducing polypharmacy risks
Coordinate care transitions between hospital, rehabilitation, and home care settings
Average admissions: 10
No trauma designation

Support Staff

Medical Assistant (MA)
Registered Nurse (RN)

Ideal Candidate

An ANCC or AANP-certified Nurse Practitioner with an active Arizona or NP Compact license who has experience in palliative geriatrics and is comfortable managing complex geriatric patients in an outpatient clinic setting.
