divh2Advanced Practice Nurse - Bridge Care Bergen County/h2pGreat new Career Opportunity in Palliative Care!/ppThe Advanced Practice Nurse will be responsible for, but are not limited to:/polliConduct visits to patients on palliative care in accordance with local, state and federal regulations./liliCommunicate visit findings to designated members of the interdisciplinary team, including the National Director of Palliative Care./liliFacilitate access to appropriate support models of care (i.e., hospice, rehabilitation, on-going aggressive treatment)./liliConsult with other members of interdisciplinary team to contribute to the patients plan of care./liliPrepare and maintain accurate patient records, charts and documents./liliAssist in development of clinical practice guidelines/standards in support of quality palliative care as needed./liliEnsure collaboration with patients, caregivers, medical directors and members of IDT./li/olpEducation:/ppMasters Degree in Nursing/ppProfessional Experience:/pulliMust have a Nurse Practitioners license in the state of operations/liliMust have 2-4 years experience working within hospice or related field/liliPalliative care experience preferred/liliMust have a Masters Degree/liliMust have a valid drivers license and a reliable source of transportation/liliMust be willing to travel/li/ulh2About Us/h2pThe CareOne mission is to define excellence within the health care community.
