Lead Navigator: Behavioral Health & Housing Outreach

Community Bridges, Inc.

Community Bridges, Inc. (CBI) is seeking a Lead Navigator to support individuals experiencing homelessness by conducting outreach and delivering wrap-around services. The role includes coordinating with RBHA and non-RBHA community-based services and mentoring Navigator II staff. The Lead Navigator will engage with high-acuity clients, manage housing referrals, and ensure smooth discharge planning within a multidisciplinary team. #J-18808-Ljbffr Community Bridges, Inc.
