Overview
Licensed Practical Nurse (LPN) – Long-Term Care in Franklin, NH. Travel assignment with Healthforce Healthcare LLC starting 06/15/2026 for 9 weeks. Weekly pay range: $1,645–$1,652 per week with 36 guaranteed hours. Shift: 3x12-hour day shifts (07:00–19:30). Setting: Skilled Nursing Facility.

Responsibilities
- Administer prescribed medications and treatments in accordance with physician orders and facility policy, and document administration accurately.
- Monitor patient status, assess vital signs, and report changes promptly to the supervising RN.
- Provide direct patient care, including assistance with activities of daily living and mobility, while maintaining dignity and safety.
- Perform wound care, dressing changes, and routine nursing procedures as delegated by the care team.
- Maintain precise documentation in patient records, including shifts notes, care planning, and medication reconciliation.
- Collaborate with licensed staff and ancillary teammates to ensure continuity of care and a safe, patient-centered environment.

Requirements
- Active Licensed Practical Nurse license in good standing in New Hampshire (or eligibility to practice in NH).
- Graduation from an approved LPN program (LPN diploma/certificate).
- Basic Life Support (BLS) certification.
- Approximately 1 year of LPN experience in a long-term care or skilled nursing setting preferred.
- Ability to work 12-hour day shifts within a rotating schedule and adapt to a short-interval rotation rhythm.
- Eligible for travel assignments in the United States and able to report to the assignment location as scheduled.
