Physical Therapist

Title: Physical Therapist
Duration: 12 months + Possibility of Extension
Location: Bourne, MA
Coverage Areas: Team A territory including Cape Cod from the Canal to East Harwich
Shift: M-F (8 AM - 4:30 PM)
On Call/Weekend Requirement: None

Key Responsibilities:
Conduct comprehensive Start of Care (SOC) assessments for home health patients.
Perform physical therapy evaluations and establish individualized treatment plans.
Deliver skilled physical therapy interventions in accordance with physician orders and evidence-based practice.
Complete routine patient visits and reassessments as required.
Evaluate patient progress and modify treatment plans to achieve optimal outcomes.
Provide education to patients, families, and caregivers regarding treatment plans, home exercise programs, safety, and disease management.
Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate care.
Supervise, mentor, and delegate appropriate services to Physical Therapist Assistants (PTAs).
Maintain accurate and timely documentation in accordance with state, federal, and organizational requirements.
Ensure compliance with Medicare, payer, regulatory, and company standards.
Meet established productivity expectations while maintaining quality patient care.

Required Qualifications:
Active and unrestricted Massachusetts Physical Therapist (PT) License.
Previous Home Health Physical Therapy experience.
Strong assessment, treatment planning, and patient education skills.
Ability to work independently in a field-based environment.
Valid driver's license and reliable transportation. Medical, dental, and vision insurance are available to qualified candidates who meet eligibility requirements.
