Position Overview We are seeking a compassionate and motivated Physical Therapist (PT) to join a well-established Skilled Nursing Facility in Casa Grande, Arizona. This is an excellent opportunity to work in a collaborative rehabilitation environment focused on helping patients improve mobility, regain independence, and enhance their overall quality of life. Key Responsibilities
- Perform comprehensive patient evaluations and develop individualized treatment plans.
- Provide skilled physical therapy services to help patients achieve functional goals and maximize independence.
- Conduct initial evaluations, reassessments, and discharge planning.
- Deliver one-on-one treatment sessions in accordance with established standards of care.
- Collaborate with physicians, nursing staff, and interdisciplinary team members to coordinate patient care.
- Maintain accurate and timely clinical documentation.
- Educate patients and caregivers regarding treatment plans, home exercise programs, and discharge recommendations.
- Ensure compliance with facility policies, state regulations, and professional standards.
- Full-Time Position
- Day Shift (8-hour shifts)
- Weekend flexibility may be required based on scheduling needs
- No on-call responsibilities
- Young Adult through Geriatric patients
Required
- Graduate of an accredited Physical Therapy program
- Current Arizona Physical Therapist License
- Current BLS Certification
- Arizona Fingerprint Clearance Card
- Previous Skilled Nursing Facility (SNF) experience
