Direct Care Worker/Caregiver

Penn Home Health Care is seeking a compassionate, dependable, and professional Direct Care Worker/Caregiver to provide one-on-one, in-home support to an adult participant in Chester Township, PA.

This is a full-time, Monday through Friday position offering a consistent schedule and the opportunity to make a meaningful difference by helping an individual remain safe and independent at home. Please apply only if you are able to reliably commute to Chester Township, PA 19013.

Responsibilities

Assist with bathing, dressing, grooming, and personal hygiene

Provide toileting and incontinence care as needed

Assist with ambulation, transfers, and mobility

Prepare light meals and snacks

Perform light housekeeping related to the participant's care

Laundry and linen changes

Provide medication reminders

Assist with activities of daily living (ADLs) and instrumental activities of daily living (IADLs)

Promote participant safety, independence, and dignity

Complete Electronic Visit Verification (EVV) and required documentation

Report changes in condition, safety concerns, or incidents promptly to the agency

Qualifications

Experience providing personal care preferred

Direct Care Worker, Caregiver, Personal Care Aide (PCA), Home Health Aide (HHA), or Certified Nursing Assistant (CNA) experience strongly preferred

Compassionate, dependable, and professional

Ability to work independently while following an individualized care plan

Reliable transportation and dependable attendance

Excellent communication and interpersonal skills

Must meet all Pennsylvania home care employment requirements
