Job Description
Provide and coordinate care by assessing physical and behavioral health needs of clients, develop and implement nursing plans, maintains medical records and educate clients and their families about various physical and behavioral health conditions while always ensuring clients safety.
coordinates total client services by conducting comprehensive assessment, monitoring the clients conditions, promoting sound preventive practices, coordinating services, teaching and training activities through regular assessment. Assist with client admission process as needed
Assess clients status on an ongoing basis and pertinent information gathered is documented
Ensures nursing treatment plans are specific to assessed needs and initial treatment plan is completed upon admission etc
