AOT Care Coordination Specialist - Health Home

Post Graduate Center for Mental Health is seeking a full-time care coordinator to manage a caseload of 12–15 AOT clients in the Bronx. You will maintain weekly face-to-face contact, coordinate with care plans, and liaise with managed care plans to support member needs. The role requires a 4-year degree, knowledge of Medicaid and entitlements, and strong advocacy and communication skills.
