Responsibilities

• Administer medications safely and accurately.

• Perform physician-ordered treatments, including wound care and respiratory treatments.

• Monitor residents for changes in condition and report concerns promptly.

• Collaborate with RNs, CNAs, therapists, physicians, and other interdisciplinary team members.

• Maintain accurate electronic health records (EHR) and documentation.

• Ensure compliance with all federal, state, and facility policies and regulations.

• Supervise and support Certified Nursing Assistants (CNAs) during your shift.

• Provide compassionate, resident-centered nursing care.

• Perform other duties as assigned.

Qualifications

• Active, unrestricted Wisconsin Licensed Practical Nurse (LPN) license required.

• Minimum of 1+ years of experience as an LPN is required.

• Long-Term Care (LTC) or Skilled Nursing Facility (SNF) experience preferred.

• Strong clinical assessment and communication skills.

• Experience using Electronic Health Records (EHR) preferred.

• Ability to work independently and as part of a multidisciplinary team.
