Home Care Physical Therapist

Schedule: 8 AM - 5 PM

The Home Care Physical Therapist (PT) supports the mission, vision, and values of the organization by demonstrating excellence, competence, collaboration, innovation, respect, and a commitment to patient and family-centered care. The PT delivers high-quality, evidence-based care through comprehensive assessment, diagnosis, outcome identification, planning, implementation, documentation, and evaluation of care effects, all in accordance with national standards of practice and applicable guidelines.

Responsibilities:

• Conduct thorough assessments and develop individualized treatment plans.
• Implement and document therapeutic interventions.
• Evaluate patient progress and adjust plans accordingly.
• Collaborate effectively with patients, families, and healthcare team members.

Required Experience / Certifications / Licensure:

• Graduation from an accredited physical therapy program.
• Current State of Maine licensure as a Physical Therapist.
• Valid driver's license and access to a vehicle, covered by appropriate liability insurance.
• Current certification in Basic Life Support for Healthcare providers.
• Minimum one year of full-time equivalent experience in a supervised setting.
• Strong interpersonal and communication skills.
• Analytical ability to identify changes in patient conditions and initiate appropriate actions.
