You will report to our Associate Medical Director.Why You'll Love Working HereTeam-Based Care Model: Partner with a dedicated care team that supports patients' physical, emotional, and social wellness.More Time with Patients: See fewer patients per day and spend more time delivering meaningful care.Supportive Culture: We foster a welcoming, inclusive environment where teamwork and growth are prioritized.Your Role & ResponsibilitiesYou will provide comprehensive care to seniors following clinical standards.You will collaborate daily with your care team to ensure coordinated, high-quality care.You will manage referrals, hospital/SNF coordination, DME, and home health services.You will document care accurately and efficiently with support from quality-based coders.You will participate in on-call rotation and support growth plans at your center.You will have passion for providing value-based care.What We're Looking ForRequired:Master's Degree in Nursing or completion of a PA program with board certification.Active, unrestricted NP or PA license in the state of practice.If selected for this role, we will screen you for TB.Preferred:Experience in value-based care, primary care or Medicare Advantage settings.Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers.Familiarity with HEDIS quality indicators and Medicare guidelines.You will work onsite at our Conviva locations as a Nurse Practitioner (NP) or Physician Assistant (PA) Float covering Jupiter through West Palm Beach.
