Bronx Case Manager: Housing & SMI Support

Beacon of Hope (BOH), a Division of Catholic Charities Community Services (CCCS), seeks Case Managers to support Bronx clients through the Supported Housing Program. You will conduct home visits, assist clients with Serious Mental Illness, and coordinate with multiple services as required by CCCS/BOH policies. Working hours are typically Monday–Friday, 9:00 AM–5:00 PM.
