As a Medical Social Worker (MSW) youll contribute to our success in the following ways:

• Assess patients to identify psychosocial financial environmental and safety needs.
• Complete initial social work evaluations and reassess patient needs during follow-up visits.
• Develop and update individualized care plans based on patient needs goals and physician orders.
• Communicate changes in patient condition concerns or safety issues to physicians and the care team.
• Connect patients and families with appropriate community resources and support services.
• Counsel and educate patients and families to help address social emotional and care-related needs.
• Coordinate with the care team to support continuity of care and effective discharge planning.
• Provide compassionate high-quality social work services that support positive patient outcomes.

What is Required

• Current State License as a Social Worker
• Masters Degree or Doctoral Degree in Social Work from a school of Social Work accredited by the Council of Social Work Education
• 1 year of social work experience in a healthcare setting
• Social work skills as defined as general social work standards
• Ability to frequently lift push pull and support up to 50 pounds including positioning or transferring patients and moving equipment
• Must possess a valid drivers license maintain an acceptable driving record OR have reliable transportation to travel between patient homes and assigned work locations.
