Position Overview

This is a contract assignment for a Registered Nurse (RN) in a hospice care setting. The assignment is for a duration of 13 weeks, with a standard schedule of Monday through Friday from 8:00 AM to 5:00 PM. The RN will provide home health services and attend periodic office meetings.

Key Responsibilities

Perform home visits to provide nursing care to hospice patients.
Complete documentation of visit types, including but not limited to Start of Care, Wound Care, and Follow-up visits.
Maintain communication with the care team and attend office meetings as required.
Support patient care coordination, ensuring timely and accurate follow-up for each patient.

Schedule and Travel Expectations

Contract Duration: 13 weeks
Work Hours: Monday to Friday, 8:00 AM - 5:00 PM
Weekend Rotation: One weekend a month
On Call: Minimal on-call responsibilities
Travel Requirements: Travel expected within the designated territory, approximately 200-400 miles per week.
Average Daily Caseload: 5-10 patients, totaling approximately 30 productivity points weekly.

Qualifications

Minimum of one year of recent experience in adult home health or hospice care.
Proficient in Homecare Homebase and OASIS. Previous experience within the last six months is preferred.
Active RN license required; pending licenses will not be accepted.
CPR and BLS certifications are mandatory.
Reliable transportation, a current driver's license, and agency-required liability insurance are essential.

Additional Information

Must be willing to travel up to 75 miles from the home clinic.
All documentation must be completed within 24 hours of patient visits.
There are no guaranteed hours; clinicians are expected to meet the weekly productivity requirement.
Local candidates will be considered at the same rate.
