Part-Time Remote Care Manager & Advocate

Care Navigators Inc

Care Navigators, Inc. and Care Guardians NFP, Inc. are seeking qualified part-time care managers to join their team in Illinois. Responsibilities include comprehensive assessments, care planning, discharge assistance, and home care coordination.

Candidates should have care management experience, strong communication, a valid driver’s license, and proficiency with Word, Excel, and G-Suite. Remote options available; license in RN/SW/LCSW recommended.
