We are a first-class inpatient treatment center offering top-level medical care and evidence-based clinical care for individuals suffering from substance use disorder . We are seeking Patient Care Coordinators to join our team. Patient Care Coordinator will be responsible for providing support to clients to reduce barriers to treatment and keeping clients engaged in addiction treatment services. They will model competency in recovery and ongoing coping skills while encouraging and inspiring hope for a client in recovery. Interact with cl ients in a professional, courteous, empathetic, and supportive way at all times . Regularly communicate with Counselors and Nursing staff regarding cl ient issues and cl ient care. Encourage positive interactions and conversations amongst cl ients and staff. Transport cli ents throughout the facility. Assure cl ients adhere to program activity schedule . Conduct room checks on all cl ients. Perform census checks every 2 hours (4 times per shift) including after meals. Observe clients for inconsistent behavior and report to appropriate staff (Clinical, Nursing, etc.) and include this information in your shift report. Assist in the de-escalation of behaviorally compromised cl ients. Monitor cli ent linen and laundry. Monitor linen supplies and replenish as needed. Ensure soiled linen is placed in laundry bin at the end of every shift. Collect laundry bags of dirty clothing and prepare for housekeeping to wash. Deliver clean laundry back to cl ients. Search and record new admissions belongings. Assist staff with screening the client and their property for contraband. Performs other duties as assigned by supervisor. Act as a role model for cl ients and peers! Minimum Qualifications: High school diploma or equivalent One (1) year of related work experience Skills: D etail oriented Great interpersonal skills Be kind - we’re here to help people and provide the best possible cl ient care Be empathetic - understand that our cl ients are here to get better Be emotionally intelligent - understand that cl ients can often be emotional over seemingly insignificant issues, and it's not personal!
