Remote Behavioral Health Case Manager

Independence Blue Cross in the Tri-State area seeks a Behavioral Health Case Manager to support members remotely. You will conduct telephonic assessments, develop care plans, and coordinate with care teams to improve behavioral and physical health outcomes. The role requires 3–5 years post-licensure in integrated care, strong SDOH and motivational interviewing skills, and comfort using telehealth tools.
