Patient Aligned Care Team (PACT) Licensed Practical Nurse (LPN)
Below are some of the primary duties and responsibilities:
- Prepares and administers prescribed medications via (oral, topical, subcutaneous, intramuscular and/or intravenous) and performs treatments according to established policies and procedures.
- Takes appropriate actions based upon communications, observations, veteran care reports, provider/staff discussion.
- Assists the RN/provider and other staff in proactively identifying and addressing veteran care needs.
- Accurately and timely records all patient data and education into the medical record.
- Knowledge and skill in performing support duties for diagnostic tests and/or other specialized equipment and monitoring the Veteran's condition before, during, and following the procedure.
Work Schedule: Monday-Friday, 8:00am-4:30pm
