Diabetes Educator – Per Diem (As needed) – Advocate Condell Medical Center, Libertyville, IL. This part‐time position requires working weekday shifts of 8‐ to 10‐hour duration (8:00am–4:30pm or 8:00am–6:30pm). Pay ranges from $35.50 to $53.25 per hour. Responsibilities Provides care following the National Standard for Diabetes Self‐Management Education and Support (DSMES) as defined by the ADA or ADCES. Assesses patient learning needs, goals, and barriers to care, and tailors educational interventions accordingly. Leads individual and group education sessions on diabetes management, including medication, nutrition, exercise, and blood glucose monitoring. Provides formal instruction on diabetes management to patients in inpatient and outpatient settings. When a registered dietitian, delivers medical nutrition therapy services for patients with diabetes or other qualifying diagnoses. Provides ongoing support and follow‐up to ensure successful management of diabetes and related conditions. Maintains current knowledge of diabetes management technology and manufacturer protocols. Collaborates with the healthcare team to develop and implement comprehensive care plans. May adjust diabetes medications per protocol under provider order and departmental policy. Stays current on developments in diabetes care and education, ensuring compliance with ADA standards. Documents and charges services consistent with CMS and payor requirements and guidelines for DSMT and MNT services. Demonstrates knowledge and skills appropriate to the age of patients served, including principles of growth and development. Participates in community events and coordinates support meetings, serving as a health resource. Develops and evaluates patient education programs and materials. Acts as a resource for health care providers, patients, and the community. Completes required hours of diabetes‐related continuing education annually if not a certified CDCES. Licensure & Certification Current license or certification to practice as a Registered Nurse (RN) or Registered Dietitian in the applicable state. Certified Diabetes Care and Education Specialist (CDCES) preferred; if not certified, must obtain certification within two years (preferred) and no more than three years. Education Degree in Nursing, Dietetics, or other related disciplines that meet the certification eligibility requirements for CDCES. If a nursing degree, a BSN is preferred. A master's degree in Dietetics is required if a dietician license is obtained after January1,2024. Experience At least two (2) years of patient‐oriented educational experience, with a broad knowledge of diabetes across age‐appropriate populations and its interrelation with co‐existing health conditions. Public‐speaking skills or potential to develop for group/education classes or professional speaking opportunities. Self‐directed approach.
