Job Opportunity – Home Health Registered Nurse – Iowa

Start Date: July 28, 2026

Location: Plymouth County, Iowa

Compensation: Details provided to qualified applicants

Schedule: Monday through Friday, day shifts, 32-40 hours per week; call rotation one in four nights, weekends, and holidays

Highlights:

• Provide compassionate, comprehensive nursing care to patients in the comfort of their homes throughout Plymouth County
• Conduct OASIS assessments and develop individualized care plans for in-home patients
• Work with Epic EMR system to document and coordinate patient care
• Flexible scheduling options available with 32-40 hour work week
• Participate in departmental call rotation to ensure continuity of care
• Join an established home health team dedicated to delivering high-quality community-based care

Requirements:

• Active Iowa Registered Nurse (RN) license in good standing
• Minimum 2 years of experience performing OASIS assessments
• Demonstrated proficiency in care planning for in-home patient populations
• Previous experience with Epic EMR is preferred
• Valid driver's license and reliable transportation for home visits
• Strong clinical assessment, communication, and organizational skills
