Registered Nurse (RN) - Home Care

Job Summary


Our client is seeking a compassionate Registered Nurse to provide skilled nursing care to participants in their homes while collaborating with an interdisciplinary care team to support their health, independence, and quality of life. This role offers the opportunity to build meaningful patient relationships and deliver hands-on nursing care without the administrative burden of OASIS documentation, case management, or on-call responsibilities.


Key Responsibilities

  • Conduct field patient visits from hospital discharge lists and urgent post-hospital updates.
  • Participate in daily in-person or remote morning huddles to set clinical priorities.
  • Manage a target efficiency metric of 10 field patient visits per day.
  • Log patient workflows and clinical updates in the CareHub EHR system.
Qualifications
  • Active Colorado RN License and graduation from an accredited Registered Nurse program.
  • Valid driver's license, reliable personal transportation, and active automobile insurance.
Experience
  • One year of health care experience in a hospital, long-term care, or home health care setting.
  • One year of experience specifically working with a frail or elderly population.
Skills
  • Proficiency in phlebotomy, medication administration, and skilled nursing procedures.
  • Proficient in using Electronic Health Records (EHR) and effective in patient and family education.
Additional Requirements
  • Physical mobility and stamina for navigating diverse home environments and managing physically demanding tasks.

  • Paid Sick Leave (Medix provides paid sick leave according to state and local sick leave ordinances).
  • Health Benefits / Dental / Vision (Medix offers 6 different health plans: 3 Major Medical Plans, 2 Fixed Indemnity Plans (Standard and Preferred), and 1 Minimum Essential Coverage (MEC) Plan. Eligibility for health benefits is based on verifying that an average of 30 hours per week during the first 4 weeks of the work assignment has been met. If you meet eligibility requirements and take action to enroll, you will be covered no earlier than 60 days into your assignment, depending on plan selection(s)).
  • 401k (Eligible on the first 401k open enrollment date following 6 consecutive months on assignment. 401k Open Enrollment dates are 1/1, 4/1, 7/1, and 10/1).
  • Short Term Disability Insurance.
  • Term Life Insurance Plan.
