Role: Vulnerable Populations Coordinator (VPC)

Overview


The Vulnerable Populations Coordinator (VPC) plays a critical role in supporting the Healthcare Coalition (HCC) and Regional Health Department to enhance preparedness, response, and recovery efforts for at-risk populations. These populations may be disproportionately affected during disasters and public health emergencies.


Working under the guidance of the Tennessee Department of Health (TDH) and/or the Regional Healthcare Coordinator (RHC) / Emergency Response Coordinator (ERC), the VPC leads outreach, planning, training, and response initiatives aimed at reducing health disparities and advancing health equity across the region.

Key Responsibilities

• Collaborate with Healthcare Coalitions (HCCs), healthcare facilities, and community organizations to integrate vulnerable population needs into emergency preparedness and response planning
• Build and maintain partnerships with long-term care, hospice, home health, dialysis, assisted living, and behavioral health providers
• Support vaccination efforts and infectious disease response, including planning and operating Points of Distribution (PODs)
• Provide training and technical support on Healthcare Resource Tracking System (HRTS) and Patient Bed Matching (PBM); act as regional subject matter expert (SME) for PBM
• Maintain coalition contact lists, inventories, and preventive maintenance logs
• Develop and update resource directories for agencies supporting vulnerable populations (e.g., medical suppliers, food banks, housing services, NGOs)
• Lead outreach efforts and coordinate quarterly meetings with partner organizations
• Collaborate with government agencies, NGOs, and local coalitions to strengthen community engagement
• Support regional exercises and contribute to After-Action Reports and Improvement Plans
• Assist with reporting, data management, and compliance with ASPR HPP and CDC PHEP requirements
• Provide logistical and operational support during emergency activations
• Perform additional duties as assigned

Knowledge, Skills & Abilities

• Strong understanding of public health, emergency preparedness, and healthcare systems
• Knowledge of social vulnerability and access/functional needs in disaster planning
• Ability to design and deliver training programs
• Excellent communication, facilitation, and stakeholder engagement skills
• Proficiency in Microsoft Office Suite and preparedness systems (HRTS, PBM)
• Strong data management and organizational skills

Qualifications

• Bachelor's degree in Public Health, Emergency Management, Healthcare Administration, IT, or related field (Master's preferred)
• 2-3 years of relevant experience in public health, emergency management, or community outreach
• Required certifications: ICS 100, 200, 700, 800
• Preferred: ICS 300/400 (within 24 months of hire)
• Valid Tennessee driver's license with ability to travel regionally and statewide

Work Environment

• Hybrid role with office, community, and statewide travel
• Requires occasional lifting (up to 30 lbs) and a mix of sitting and standing
• Availability for after-hours emergency response and extended shifts during activations
• Moderate exposure risk during public health emergency operations

Work Schedule

• Days: Monday - Friday
• Hours: 7.5 hours/day
• Shift: 8:00 AM - 5:00 PM (CST)
