Nurse Practitioner, Primary Care

Provide in-home primary care to high-acuity patients See approximately 6-9 patients per day with 30-45 minute visits Manage a patient panel across Denton, Aubrey, Lewisville, and Carrollton areas Focus on patient care rather than visit volume Take the time needed to address patients’ needs Practice within an integrated, team-based, physician-driven value-based care model Work Monday-Friday with no weekends or evenings Perform home-based primary care with minimal call responsibilities Requirements Licensed as a Nurse Practitioner in the state where the HarmonyCares Medical Group office is located Federal DEA license or eligibility Active CPR/BLS certification Board certification Valid driver’s license and good driving record Directly related experience in home health, geriatrics, hospice, or primary care Geriatric training/experience preferred Core Competencies Demonstrates expertise in providing in-home primary care to high-acuity patients, with a focus on patient-centered care and effective management of a patient panel.
