Job Title: Home Health Physical Therapist Profession: Therapy/Rehabilitation Specialty: PT Home Health Duration: 13 weeks Shift: Day 5x8-Hour Hours per Shift: 08:00 - 16:30 Experience: 1 year of skilled home health experience License: Requires state license Certifications: BCLS required Must-Have: OASIS experience Description: Home Health Physical Therapist position available in a home health setting. The therapist will be responsible for a 1:1 patient care ratio. Visitation may include up to 6 patients per day. The standard expectation is to earn 30 productivity points. Travelers are accepted with prior experience. There are no weekend requirements for this position. The facility does not allow 4x10 or 3x12 shifts. The primary series COVID vaccination plus one booster is required. The position entails daily client visits, with a maximum driving radius of 60 miles. The facility effectively minimizes drive and windshield time. Business casual attire is expected, with scrubs allowed but not required. Charting will be done using remote EPIC and OASIS systems. Experiences in billing, lifting, transfer, and scheduling appointments are preferred. Additional skills related to home health, such as adaptive devices and patient education, are highly valued. The selected candidate should have reliable transportation. References are required prior to the start of employment. All travelers are expected to float between facilities within the state. Refusal to float may result in contract termination.
