Registered NurseThe Registered Nurse (RN) is responsible for providing comprehensive patient-centered care through clinical assessments, care coordination, patient education, and ongoing monitoring of patient conditions. This role works collaboratively with interdisciplinary healthcare teams and caregivers to ensure high-quality care and positive patient outcomes in the home health setting.ResponsibilitiesPerform comprehensive evaluations of patients' physical, emotional, and overall health statusDevelop, implement, and update individualized care plans based on patient needs and physician recommendationsEducate patients and caregivers on chronic disease management, medications, side effects, and self-care techniquesCoordinate care with physicians, therapists, social workers, and other healthcare professionalsMonitor patient progress and communicate changes in condition to the appropriate care team membersMaintain accurate and timely EMR documentation including assessments, interventions, and patient progress notesSupervise Structured Family Caregivers, Home Care Aides, and other caregiving staff to ensure quality care deliveryEnsure compliance with care plans, agency policies, and healthcare regulationsQualificationsActive Registered Nurse (RN) license in the applicable statePrevious home health or case management experience preferredStrong clinical assessment and patient education skillsKnowledge of EMR documentation and care coordination processesExcellent communication, organizational, and interpersonal skillsAbility to work independently and collaboratively within an interdisciplinary teamSkills:communication (Required - 10 years)computer skills (Required - 10 years)Education:Bachelor's Degree - Nursing (BSN) (Desired)Associate's Degree - Nursing (RN to BSN) (Required)High School Diploma (Required)
