We believe in taking care of the whole person - not just their health, but your overall well-being - including their family.**Job Title:** Team Physician**Department:** Clinical**Time Commitment:** Full-time**Reports To:** Medical Director**FLSA Status:** Exempt**Location:** Bellefontaine, OH; Urbana, OH; Russell's Point, OH (Choice of)**Summary**CHWP is an integrated primary care community health center that utilizes a team-based care model, sharing patient-care responsibilities among members of a team. In addition to providing relevant direct and appropriate clinical care, treatment and diagnosis to patients within the scope of practice for a primary care physician, the Team Physician functions as a Team Leader in overseeing and directing the full operations of a team-care model, providing whole-person, quality care. Function as a fully accountable colleague with nurse practitioners through collaborative care agreements, and strives to support CHWP’s quality initiatives and requirements necessary and relevant to a Federally Qualified Health Center, and various certification programs, i.e. HRSA, PCMH, Joint Commission, etc. **Primary Accountabilities***Achieve Results** Directly affect improvements in patient well-being through accurate and timely diagnosis and delivery of relevant, high quality medical care. Ensure the delivery of competent, accurate, medical care and treatment to all patients as assigned.* Provide leadership and direction to all team members and ancillary staff. Affect overall clinic results. Meet or exceed all measurable standards of care, including overall clinical, quality, and productivity goals.* Ensure accuracy, efficiency, and appropriate confidentiality with patient charting and related recordkeeping and administrative functions as assigned.*Operational Excellence** Ensure the delivery of all medical treatment and care is consistent with all regulatory, accreditation, and professional standards, including clinical policies and quality initiatives.* Understand and ensure all practices and procedures related to the delivery of medical services are consistent with contracted protocols and procedures.* Serves as the Team Lead to manage and implement the full operations of the team-care model; engaging all team members input and expertise related to the delivery of whole-person, integrated primary care.*Relationship Management** Develop and ensure favorable relationships with all patients and their families. Achieve commitments from patients to adhere to an effective plan of care, and ensure all patient services are delivered in a caring and professional manner.* Develop and ensure ongoing, positive relationships and collaboration with other members of the Team and CHWP staff. Ensure the success of each clinic operation through collaborative support and working relationships with a team approach with all CHWP staff, vendors contractors and/payer sources.* Serves as a mentor and coach to team members/staff resulting in a cohesive approach to providing integrated care services to CHWP’s patients.*Professionalism & Stewardship** Ensure all actions, job performance, personal conduct and communications represent the organization in a highly professional manner, at all times.* Uphold and ensure compliance and attention to all CHWP’s policies and procedures and quality initiatives, at all times.* Practice patient care within the scope of a nurse practitioner for primary care or related specialty* Serve CHWP’s Mission by providing quality, whole-person, patient-centered medical care to anyone and everyone in our community.* Aspire to CHWP’s Vision to change lives within our community by eliminating barriers and providing a standard of healthcare that improves the well-being of the whole person.* Commit to the Values of CHWP (BLESSED):**B**e welcoming**L**isten with heart**E**ducate patients**S**erve with excellence**S**hare best practices**E**liminate barriers**D**evelop relationships* Participate in stewardship opportunities within the organization at a minimum of twice a year.* Compliant with HIPAA.**Primary Tasks and Duties*** Comprehensive physical assessment; evaluating, diagnosing, and treating new and existing patient's medical conditions.* Initiating and interpreting labs and diagnostic studies.* Performing medical procedures (as credentialed)* Prescribing and referring patients for specialized consultation.* Write/Dictate progress notes on patients' charts indicating patient status and treatment procedures performed.* Conducting follow-up patient care.* Providing health education to patients and families.* Supervising and/or coordinating the activities of patient care and team care.* Provides management with periodic reports of the team-care model progress.* Makes valuable contribution to team objectives and expects all team members to make maximum contributions.* Teaching and training illness prevention.* Actively participate in community health education.* Performing emergency life-saving procedures in cases such as cardiac arrest, respiratory arrest, massive hemorrhage, or similar emergencies.* Are among front line medical providers in emergency disaster services.Supervisory ResponsibilitiesDirect/Indirect**Work Environment**The work environment characteristics are those for a normal medical office environment. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Work may require occasional weekend and/or evening hours. Outpatient call on an equal basis from home.Work hoursFull-time or part-time, exempt salary position with Employee Agreement. Generally, a 50-60 hour work week is required for full-time; part-time as agreed upon by parties; hours are subject to change. Office hours vary by site, but are usually 8:00am to 6:00 pm, Monday through Friday. Some Saturdays and after business hours work may be required. Assigned meetings and travel are a part of the position.WagesStarting wages for physician will be negotiable based on Community Health & Wellness Partnerspay policies and factors such as education and experience, national and state average compensation recommendations based on location and region. Employment compensation and productivity will be described in the Employment Agreement.Introductory PeriodIdentified in the Employment Agreement. Credentialing ProcessPhysician will complete credentialing process with CHWP as to qualification prior to date of hire. Qualifications and Requirements*Education** Graduate from a ACGME accredited residency program in area of specialty* Award of Medical Doctor (MD) or Doctor of Osteopathic (DO) degree*Certificates and Licenses** Valid Ohio Driver's license* State Medical Board of Ohio* Board certification within specialty* BCLS or ACLS Certified*Professional** Demonstrates excellence in a critical leadership role and displays a high level of leadership experience.* Is a strong team builder; capitalizing on talents of all team members* Demonstrated expertise in relevant medical practices, protocol, trends and best practices in clinical areas assigned.* Demonstrated knowledge and success in effecting overall clinical operations.* Experience/Knowledge of accreditation processes and requirements, as well as all federal, state and local regulations and standards associated with the delivery of care in a community health center environment.* Ability to manage multiple responsibilities and emergency situations successfully* Proficiency in knowledge of office management, use of computer, software packages, and office machines*Physical/Environmental** Occasional lifting of 40 – 50 lbs and pushing of 5-20 lbs* Sitting, standing and walking approximately 80% of the time* A medium to high level of manual dexterity required* Bending and reaching approximately 20% of the time* Normal accessibility and mobility throughout the region required* Normal overtime/extended work hours #J-18808-Ljbffr
