RN - Case Management

Hospice Registered Nurse

Monday to Friday, 8 AM to 5 PM, with occasional back-up for weekends and on-call.

As a Hospice Nurse, you will be pivotal in helping patients identify their personal goals of care and supporting caregivers in confidently managing their loved ones' needs. You will be a generalist on various diseases and a specialist in symptom management, which requires excellence in assessment and critical thinking. The Primary Registered Nurse manages an assigned caseload of patients and leads the interdisciplinary team caring for those patients. Our care is provided wherever our patients call home, including private residences, nursing facilities, group homes, and assisted living centers.

Responsibilities:

• Provide compassionate end-of-life care to patients and families.
• Assist patients in identifying personal care goals.
• Support and educate caregivers in managing their loved ones' needs.
• Lead the interdisciplinary team to provide comprehensive care for assigned patients.
• Deliver symptom management and support excellence in care assessment.

Required Experience / Certifications / Licensure:

• Active RN License.
• Strong clinical skills.
• Experience in hospice or palliative care is preferred.
