Key Responsibilities

• Launch new cases and model bestpractice care
• Provide handson personal care and ADL support
• Complete homesafety checks and qualityassurance visits
• Train, coach, and onboard caregivers
• Participate in oncall rotation and fill urgent shifts
• Problemsolve changes in client condition
• Assist with delivering supplies and documentation to field staff

What You Bring

• At least 2 years of experience supporting individuals with functional disabilities and a current CNA certificate (leadership experience a plus)
• Strong communication and interpersonal skills
• Reliable transportation and the ability to lift/transfer clients
• Comfort using mobile apps/EMR systems
• Compassionate, dependable, and calm under pressure
• Valid driver's license and auto insurance
• Reliable vehicle in good working order
• Ability to tolerate a variety of home environments
• Ability to manage physical demands, including standing or sitting for extended periods
• Willingness to work every other weekend and share oncall responsibilities
