* Provide direct patient care to residents in a skilled nursing setting, including administration of medications, wound care, and vital sign monitoring
* Develop and implement individualized care plans in collaboration with the interdisciplinary team
* Conduct thorough assessments, document patient progress, and communicate effectively with residents, families, and colleagues
* Collaborate with physicians, therapists, and other healthcare professionals to optimize patient outcomes
* Maintain accurate and detailed records of patient care and treatment
* Provide emotional support and comfort to residents and families
* Participate in quality improvement initiatives to continuously improve patient care and outcomes
