Occupational Therapist (OT) - Skilled Nursing Facility

Location: Port Charlotte, FL

We're seeking an experienced Occupational Therapist (OT) to join a reputable Skilled Nursing Facility in Port Charlotter, FL. This is a full-time, permanent role for an OT who is comfortable working in a SNF / long-term care environment and treating a geriatric and post-acute population.

Position Overview

The Occupational Therapist will evaluate residents and provide skilled therapy services focused on improving independence with activities of daily living, functional mobility, and safety.

Key Responsibilities

• Conduct occupational therapy evaluations and develop individualized treatment plans
• Provide skilled OT services to short-term rehab and long-term care residents
• Address ADLs, functional mobility, cognition, and adaptive equipment needs
• Collaborate with PTs, SLPs, nursing staff, and physicians as part of the care team
• Document evaluations, treatments, and progress in compliance with facility and regulatory standards
• Participate in care planning and discharge planning as appropriate

Qualifications

• Active Florida Occupational Therapist license
• Prior experience in a Skilled Nursing Facility or Long-Term Care setting is a plus
• Strong documentation and time-management skills
• Ability to work independently in a fast-paced clinical environment
• Experience with geriatric and post-acute rehab patients preferred
