Job Details Level: Experienced Job Location: Yukon (OK) - Yukon, OK 73099 Position Type: Full Time Job Shift: Day Job Category: Health Care FULL-TIME REGISTERED NURSE CASE MANAGER position for Yukon Home Health Office. Responsibilities Supervise and manage all care given to assigned case management patients. Oversee nursing staff that are assigned to patients as ordered by physician. Coordinate work schedule to fit patient's time and needs. Complete patient visit documentation and submit electronically using WellSky (Kinnser) EMR system. Qualifications Current Registered Nursing License approved by the Oklahoma Nursing Board of Regents. At least 1 year experience in either home health setting or acute-care setting with experience in elderly, middle-aged and/or pediatric care. Able to lift a minimum of 20 lbs without difficulty. Must own a vehicle in which to make visits and maintain current automobile insurance coverage.
