Home Health Registered Nurse

Role Overview: A Home Health Registered Nurse provides personalized, one-on-one care in patients' homes, collaborating with physicians and healthcare professionals to develop and implement individualized care plans, monitor patient conditions, educate families, and oversee home health aides. Key Responsibilities Administer medications, perform wound care, and conduct patient assessments onsite. Develop and update care plans in coordination with healthcare team. Monitor patient progress and report changes to physicians and managers. Educate patients and families on disease management and treatments. Maintain accurate care records and coordinate with healthcare providers. Supervise and ensure proper documentation by Home Health Aides. Qualifications And Skills Experience in Med surg, ICU, ER, or acute care settings. Current, unrestricted RN license and CPR certification. Ability to collaborate effectively with healthcare teams. Valid driver’s license, auto insurance, and reliable transportation. Preferred: one year nursing and home health experience. Additional Details: The role is patient-facing, requires TB screening, and adherence to driver safety policies. Compensation ranges from $77,200 to $106,200 annually, based on skills and location.
