• Shift 1: 4:00 PM – 12:00 AM
• Shift 2: 12:00 AM – 8:00 AM

Work Locations

• Hilda Barg Homeless Prevention Center: 14945 Jefferson Davis Hwy, Woodbridge, VA
• Ferlazzo Government Center: 15941 Donald Curtis Drive, Woodbridge, VA 22191

Nature of Work

• Provides case management through assessment, planning, facilitation, care coordination, evaluation, and advocacy.
• Addresses individual and family needs related to physical health, mental health, behavioral health, developmental or intellectual diagnoses, and family stability.
• Serves as a lead worker to lower-level staff.
• Manages individual and family needs within human services programs.
• Performs risk assessments and permanency planning.
• Conducts client visitations.
• Represents the department at community funding meetings.

Typical Class Responsibilities

• Interviews individuals and families to gather information regarding social, health, emotional, and economic needs.
• Assesses the needs of individuals and families.
• Provides support and guidance to subordinate human services staff.
• Assists staff with court paperwork and documentation.
• Reviews documentation to ensure program compliance.
• Investigates allegations of abuse, neglect, and financial exploitation.
• Conducts home visits and assesses client safety.
• Consults with peers regarding caseload management and appropriate staffing.
• Makes caseload service and placement recommendations.
• Participates in program planning and evaluation meetings related to program changes, funding, and evaluation.
• Performs other duties of a similar nature or level.

Minimum Education, Training & Experience

• Bachelor’s degree required.
• Two years of related experience required.
• Valid driver’s license required.
