SALARY: $140K - $162K, DOE $10,000 signing bonus Benefits: medical, dental, vision, chiro, life insurance, EAP and 403B Retirement Plan DESCRIPTION As a member of an interdisciplinary health care team, the Nurse Practitioner is responsible for providing complete clinical care for patients of all ages using a patient centered medical home model of care. RESPONSIBILITIES Under the direction of the Medical Director and per written protocol provides care consistent with community standards and follows the rules of the various payors. Confers with primary care physicians or specialists regarding the care of patients as well as medical and diagnostic work performed. Participates in Quality Assurance Initiatives and provides input on the development of policies and procedures, which impact the health delivery service. Participates in training relevant to the education of professional health personnel as well as any training required by the various funders. Provides exemplary customer service. May function on a rotating basis as the provider on-call during after-hours. Maintains the security and privacy of all Protected Health Information. Documents all information accurately and timely using an Electronic Medical Record System. Participates in all safety programs, which may include assignment to an emergency response team. QUALIFICATIONS Graduate of an accredited Nurse Practitioner Program. Current license in the State of California as a Nurse Practitioner. Must possess current DEA license and CPR (BCLS/ACLS) certificate. Strong communication skills in English and the ability to speak Spanish is preferred.
