Nurse Practitioner/ Physician Assistant (Acute-Care)
Job Type: Full-Time
Location: Warren, RI
Schedule
• 4-5 weekdays per week
• Flexible scheduling
• No weekends mentioned
Position Summary
We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to provide primary care services to patients in Skilled Nursing Facilities (SNFs) and Assisted Living Facilities (ALFs). The provider will manage patients' overall health, perform assessments, complete medication reconciliations, and provide ongoing medical care.
Responsibilities
• Conduct primary care visits and patient assessments.
• Review patient medical histories and records.
• Perform medication reconciliation.
• Conduct annual wellness visits.
• Diagnose and manage acute and chronic conditions.
• Order and interpret diagnostic tests.
• Prescribe medications and develop treatment plans.
• Educate patients and families about conditions, medications, and preventive care.
Qualifications
• Current NP or PA license.
• Required experience as an NP/PA in acute care.
• Skilled Nursing Facilities (SNF) or Assisted Living experience or Primary care/general practice experience required
• Ability to work independently and manage multiple patients.
