- Master's degree or higher
- Clinical Experience (2+ years)
- 1+ years of experience in ANY of the following:
- Hospice
- Palliative Care
- Home Health
- ALL of the following valid licenses/certifications:
- Nurse Practitioner (NP) in Pennsylvania (PA)
- Registered Nurse (RN) in Pennsylvania (PA)
- ANY of the following valid licenses/certifications:
- American Heart Association Basic Life Support (AHA BLS)
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)
Job Summary
We are seeking a compassionate and highly skilled Palliative Nurse Practitioner to join our dedicated healthcare team. In this vital role, you will provide expert symptom management, emotional support, and comprehensive care planning for patients with serious, chronic, or life-limiting illnesses. Your focus will be on enhancing quality of life through holistic, patient-centered approaches while collaborating closely with interdisciplinary teams. This position offers an exciting opportunity to make a meaningful difference in patients' lives by delivering specialized palliative care across diverse settings, including inpatient units, outpatient clinics, and home care environments.
Duties
- Conduct thorough patient assessments, including vital signs, symptom evaluation, and physiological examinations to determine care needs.
- Develop and implement individualized palliative care plans that address pain management, symptom control, psychosocial support, and advance care planning.
- Administer medications such as opioids and other symptom-relieving agents following best practices and regulatory standards.
- Coordinate with physicians, social workers, chaplains, and other healthcare professionals to ensure comprehensive patient support.
- Utilize electronic health record (EHR) systems like Epic and eClinicalWorks to document clinical encounters accurately and maintain detailed medical records.
- Provide education to patients and families about disease progression, treatment options, medication administration—including injections and IV infusions—and end-of-life care preferences.
- Participate in clinical research initiatives related to hospice and palliative medicine to advance evidence-based practices.
